I take full responsibility for my experience. I understand that Family Constellations and Somatic Healing can be emotionally intense. I understand that this session and future sessions may bring up issues of a highly personal nature that may cause me to experience emotional or physical responses that may be unexpected and\/or unpleasant.<\/p>\n

Further, I understand that I may experience mental, emotional, physical, or spiritual distress and that such distress may cause unpleasant feelings. I agree to assume this risk, including but not limited to the types of responses described.<\/p>\n

I do not currently suffer from any major mental or physical impairment, and have not been diagnosed in the past with any disorder, condition, or injury, either physical or mental, that would make it unadvisable for me to assume such risks.<\/p>\n

I acknowledge that Socorro Diaz is not a therapist or physician licensed in California. She provides professional services in accordance with her training and experience, in support of her clients. Constellations and Somatic sessions are not designed as a replacement for therapy with a mental health care professional or as a substitute for any other professional consultation.<\/p>\n

This session is designed as an educational experience only. I willingly agree to the preceding statements and to hold harmless and release from all liability Socorro Diaz, and I consent to participate in a Family Constellations or Somatic session.<\/p>\n
